Wraparound Care Coordination for Families in Part C The purpose of the Wraparound Care Coordination (WCC) for Families in Part C grant is to develop intensive supports and care coordination to enrolled Part C families. WCC uses a team-based planning and high intensity care coordination process that ensures children and their families receive individualized, community-based services and natural supports. WCC facilitators will work with families to develop and implement service plans that address the priorities and needs of both parents and children. To make certain that the Wraparound services provided to families are faithful to WCC, facilitators will be trained in the Wraparound process through the use of manualized training and ongoing consultation.
